Privacy Policy
Effective date: May 30, 2026
1. Introduction
Threshold Health & Performance ("Threshold," "we," "us," or "our"), operated by Dr. Lars Health Consulting Inc., is a premium physical therapy and performance practice based in Virginia. We provide clinical care, remote coaching, performance consulting, strength and conditioning guidance, mobility and recovery programming, and educational content to clients and prospective clients across the United States.
This Privacy Policy explains what information we collect, how we collect it, how we use and protect it, and the choices you have. It applies to thresholdhp.com (the "Site"), our intake and scheduling tools, and our email and text-message communications. By using the Site or providing your information to us, you agree to the practices described here.
2. Information We Collect
We collect the following categories of information:
- Identity and contact information: your name, email address, telephone number, and mailing address. - Health and performance information: information you choose to share about your symptoms, injuries, history, training, and goals so we can deliver appropriate care and programming. - Scheduling and service information: appointment details, service selections, intake responses, and communications with us. - Payment information: processed by our third-party payment processor; we do not collect or store full payment card numbers on our own systems. - Technical and usage information: IP address, browser and device characteristics, pages viewed, and referring sources, collected automatically through cookies and similar technologies.
3. How We Collect Information
We collect information when you:
- Submit a contact, lead, intake, or scheduling form on the Site; - Communicate with us by phone, email, or text message; - Book or attend an appointment or enroll in a program; - Make a payment; or - Browse the Site, where cookies and analytics tools collect technical and usage data automatically.
4. SMS Consent and Collection
When you provide your mobile telephone number and affirmatively opt in, you give express written consent to receive text messages from Threshold. We collect SMS consent through clearly labeled checkboxes on web, lead, intake, booking, and contact forms, during client onboarding, and through documented verbal consent. Consent to receive marketing text messages is not a condition of purchasing any product or service.
Message types may include appointment reminders, follow-ups, intake coordination, service updates, educational content, and promotional offers. Message frequency varies based on your interactions with us. Message and data rates may apply. You may opt out at any time by replying STOP, and you may request assistance by replying HELP. See our SMS Communications and Consent Policy for full details.
5. How We Use Your Information
We use the information we collect to:
- Provide, schedule, coordinate, and deliver our services and care; - Communicate with you about appointments, programs, and inquiries; - Send transactional, conversational, and, where you have consented, marketing messages by email and text; - Process payments and maintain accurate business and clinical records; - Operate, secure, analyze, and improve the Site and our services; and - Comply with our legal and regulatory obligations.
6. Data Retention
We retain personal information for as long as necessary to provide our services, maintain required clinical and business records, comply with legal obligations, resolve disputes, and enforce our agreements. When information is no longer needed, we dispose of it using reasonable measures.
7. Cookies and Analytics
The Site uses cookies and similar technologies to operate the Site, remember preferences, measure performance, and understand how the Site is used. You can control cookies through your browser settings; disabling certain cookies may affect Site functionality. We may use analytics providers that process usage data on our behalf.
8. Third-Party Platforms and Service Providers
We use trusted third-party platforms to operate our business, including a customer relationship management and messaging platform (GoHighLevel), scheduling tools, email and SMS delivery services, and analytics providers. These providers process information solely to perform services for us and are bound by confidentiality and data-protection obligations. We do not authorize them to use your information for their own marketing purposes.
9. Payment Processing
Payments are processed by Stripe, a third-party payment processor. When you make a payment, your payment card details are collected and processed directly by Stripe under its own terms and privacy policy. We receive limited transaction information (such as confirmation and the last digits of a card) and do not store full card numbers.
10. Email and SMS Communications
We send email and text-message communications for transactional and, where consented, marketing purposes. Every marketing email includes an unsubscribe mechanism. You can opt out of text messages at any time by replying STOP. Opting out of marketing communications will not affect transactional messages necessary to deliver services you have requested, such as appointment confirmations.
11. Opt-Out Rights
You may opt out of marketing emails using the unsubscribe link in any such email, and you may opt out of text messages by replying STOP to any message. You may also contact us using the details in Section 19 to update your communication preferences.
12. Mobile Information Handling
We treat mobile telephone numbers and SMS opt-in data with particular care. This information is used only to deliver the messaging you have consented to and to support that service.
13. No Sale or Sharing of Mobile Information for Marketing
Mobile information will not be sold or shared with third parties or affiliates for marketing or promotional purposes. No mobile information or SMS opt-in data is shared with third parties for their independent marketing. Information may be shared only with subcontractors that directly support our messaging operations (for example, our CRM and SMS delivery providers), solely to provide the service and subject to confidentiality obligations.
14. Health Information Notice (HIPAA-Adjacent)
Some services we provide are physical therapy services, and certain information you share is health-related. Where we act as a covered entity or business associate under the Health Insurance Portability and Accountability Act (HIPAA), we handle protected health information in accordance with applicable law and any separate Notice of Privacy Practices we provide. Many of our performance, coaching, and educational offerings are wellness services that may not be subject to HIPAA. This Privacy Policy does not itself create any HIPAA rights or guarantees, and where a separate Notice of Privacy Practices applies, that notice governs the handling of protected health information.
15. Data Security
We maintain reasonable administrative, technical, and physical safeguards designed to protect your information against unauthorized access, use, alteration, and disclosure. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
16. Your Rights
Subject to applicable law, you may request to access, correct, or delete your personal information, and you may opt out of marketing communications. To exercise these choices, contact us using the details in Section 19. We will respond within the time required by law.
17. Jurisdiction
We operate from the Commonwealth of Virginia, United States, and the Site and our services are intended for users located in the United States. By using the Site, you consent to the processing of your information in the United States in accordance with this Policy and applicable Virginia and federal law.
